- FOR ALL APPOINTMENTS: In order to book your appointment a credit card will be required to secure your session. In the event that you book your appointment and do not show or cancel, your card will be charged automatically $25. Once you Book Online you will receive an email shortly after, it will be your client form, General Release of Liability Form and medical history form. Please check your spam folder incase it’s not in your inbox. Forms are mobile friendly. Forms are due upon receipt. All services require Credit Card information to be entered in to hold your Appointment as Part of our Policy. There is No Exception for this. Your Card is not charged nor is there a hold on your card unless you are to cancel the same day or do not show to your Appointment. You will than be charged $25 to rebook. At the day of your appointment if you wish to pay with another card or cash you can or change your method of payment.
